Kralkatorrik is in the crystal desert. It's the crystal dragon that was asleep in the blood legion homelands, woke up, then caused the brand by simply flying down to the crystal desert (where it was going to confront Glint, its former champion (like the Shatterer), and Destiny's Edge, who would have killed it if Logan hadn't ran off, hence why DE disbanded in the first place).
It's likely that Palawa Joko and Kralky are fighting each other, since Joko really doesn't like to share.
Primordus is, as far as anyone else knows, still in the Depths of Tyria. Jormag is in the Far Shiverpeaks, Bubbles is somewhere in the sea pissing off the karka, and Mordy is probably still asleep in the Maguuma Jungle.

Edit: I added a link to a version that is constantly updated by the guy who made the initial map. Less forum lag from massive pics as well.

Also at some point we have to go fight Primordus. And theres a convenient giant hole in the ground right in the middle of Divinity's Reach. Underground map? Dungeon? Please?

Further on that point most of the Deldrimor Front is still undiscovered in GW2. Whatever is going on there..

DR's hole exists only because they yanked the Canthan District. Was due to potential backlash in Asian markets.
Otherwise, yes, it could be incredibly convenient for going after Primorius.

Considering his lair was near/in the Central Transfer Chamber, he probably is still there.

His lair was actually directly underneath the Central Transfer Chamber. Vekk said in Eye of the North that they built the Central Transfer Chamber there because of the high levels of magic present there (from the elder dragon, although unbeknownst to them at the time of construction). While it is possible that Primordus is still in that location, I feel it is pretty unlikely for a few reasons.
Firstly, I feel the Claw of Jormag is a huge detractor of Primordus still being in that spot. I remember reading somewhere that the dragons don't get too close to one another and basically respect each other's space. If that is the case, I highly doubt that Jormag would let one of his lieutenants too close to the area which another elder dragon is hanging out.
Secondly, there aren't any real indicators that Primordus is still in the area. No matter where we go, we see proof of the elder dragon's presence. Krakaltorrik has the branded, Zhaitan, the undead, Jormag has the corrupted creatures and sons of svannir. If Primordus were still in this area, we would most likely see destroyers and possibly a lieutenant or two in that area. The closest we get to any Primordus influence in the area is a theory that the Flame Legion are working with/for him in the same way that the Sons of Svannir are working for Jormag. The only places where we actually see evidence of destroyers in game right now are in Kessex Hills, Metrica Provence, and Brisban Wildlands as well as a couple locations near Dredge deep dig locations. This implies that Primordus is underground somewhere possibly to the south and western part of the map. The truth is that the depths of Tyria are rather expansive and they could be just about anywhere.
