Also, we know that the dragons can create new lieutenants because of Edge of Destiny. In that book, a lieutenant of Jormag is killed, as are several other lieutenants throughout the book.
The megadestroyer and other destroyers in Mount Maelstrom and in the Asura areas makes me feel that Primordus might be somewhere near the old EotN Dungeon Shards of Orr. The tunnels there cut clear from one side of the map to the other while crossing the sea of sorrows south of Southsun Cove. If the dragon is not in that particular spot, it could really be anywhere thanks to dredge tunnels. There is another possibility though.
The Inquest has been taking dragon minions in an effort to harness their energies. The Crucible of Eternity was all about extracting elder dragon powers and weaponizing it. Since the Megadestroyer spawns in a volcano with an inquest base in it, it is not unnatural to believe that the Inquest could have brought the megadestroyer there.
